School Physical with Required Immunizations - State of Illinois form
NEW STUDENTS:
New students to Queen Bee coming from outside Illinois or students entering a school system for the first time: Illinois law requires proof of physical and vision examinations performed within the year preceding enrollment and a written record of required immunizations must be provided prior to starting school. Proof of a dental examination is also required.
Preschool/Early Childhood Requirements:
Illinois law requires proof of a physical examination performed within one year of starting school and a written record of required immunizations must be provided for NEW Preschool/Early Childhood students prior to the start of school.
Kindergarten:
Illinois law requires proof of physical and vision examinations performed within one year preceding the start of school and a written record of required immunizations must be provided prior to starting school. Proof of a dental examination is also required.
Second Grade Requirements:
Illinois law requires a dental examination must be submitted for ALL 2nd grade students.
Sixth Grade Requirements:
Illinois law requires proof of a physical examination performed within one year preceding the start of school and a written record of required immunizations must be provided for ALL 6th grade students prior to starting school. A dental examination is also required for all 6th grade students.
